One of the following valid proofs of identity will be required by you: Massachusetts Driver’s License, Massachusetts State ID, Massachusetts Commission for the Blind (MCB) ID, US Military ID, and US Passport. In case you possess a military ID or a US Passport as your form of proof of identification, then you will have to present an additional Proof of Residency document.

Proof of Residency

Valid proof of Massachusetts residency can include any document that provides evidence of your current residential address. These include utility bills, bank statements, the current property tax bill, voter registration card, and health insurance documentation.

A note on file requirements: Make sure that when you upload your picture in the Online Medical Use of Marijuana Application system, it should have a minimum resolution of 400 pixels in width by 360 pixels in length; it should be a .jpg file, and its size should not be more than 2 MB. All other documents should have a size of no more than 2 MB.

Step-by-Step: How to Complete Your Massachusetts Medical Marijuana Card Application

Step 1:

Scan and store all of the required documentation on your computer prior to beginning the process. In case you are uploading your U.S. Military ID, scan the front and the back of the ID and save it as one file. Ensure that your photograph and all other required documentation adhere to the file specifications listed above.

Step 2:

Visit the Medical Use of Marijuana Online System at patient.massciportal.com.

  • Click on the “Create Account” link.
  • Enter your information in the “User Information” section.
  • Accept the Terms and Conditions by checking “I agree to the Medical Use of Marijuana Online System’s terms and conditions.”
  • Complete the CAPTCHA verification.
  • Click “Create Account.”

You will receive a confirmation email at the email address you have given us. Once you receive it, click on the link in the email to validate the email address and finish the registration process. When you click on the link, you will be taken to the login screen. Please note down your username and password because you will require them for renewal.

Step 3:

Enter your username and password and click “Login” to access the system.

Step 4:

Click the “Register as an Adult Patient” button.

Step 5:

Input all four ID fields and click “Proceed”. Your PIN is the number that was provided to you by your MMJ Medics certified healthcare provider. This number was also emailed to you by the system following certification.

Step 6:

If all four identification fields are entered successfully, you will be taken to the Instructions page. Read through the instructions carefully and click “Proceed” to continue.

Step 7:

Fill in all required fields, which are marked with a red asterisk (*). These include:

  • Registration Information: Mother’s Maiden Name and Gender.
  • Residential Address: Address, City, and Zip Code. If you are homeless, you may check the box indicating your status – you will then be required to provide a Massachusetts mailing address. Important: If you are submitting a Massachusetts Driver’s License or State ID as your form of identification, the name and address on your application must match the name and address on that ID exactly.
  • Mailing Address: Address, City, and Zip Code. If your mailing address is the same as your residential address, check the box, and the fields will auto-populate.
  • Identification Document Information: From the drop-down menu, select the valid form of ID you are uploading, and enter the corresponding ID number. Depending on your document, this will be your Massachusetts Driver’s License number (include the ‘S’), Massachusetts ID number, U.S. Passport number, or U.S. Military ID number.
  • Attestation: Read through all attestations and check each box to confirm that you understand and agree. Once complete, click “Proceed.”

Step 8:

If all fields are entered successfully, you will be taken to the “Valid Form of Identification” page.

  • Enter the expiration date of your ID.
  • Click “Choose File” or “Browse” and select the file you saved in Step 1.
  • Note: Do not upload your photograph at this step – you will be prompted to do that on the next page.
  • If you are uploading a U.S. Military ID, make sure your file includes both the front and back of the card.
  • Click “Proceed” to move to the next page.

Step 9:

If you used a U.S. Passport or U.S. Military ID, you will now be taken to the “Proof of Massachusetts Residency” page.

  • Click “Choose File” or “Browse” and select your proof of residency document.
  • Click “Proceed” to continue.

Step 10:

You only need to upload a photo if you did not use a Massachusetts Driver’s License or Massachusetts State ID.

  • Read the photo requirements on this page to confirm your image meets the specifications.
  • Click “Choose File” or “Browse,” select your photo file, and click “Upload.”
  • Your photo will appear under “My Uploaded Photos” once successfully uploaded.
  • Click “Proceed” to continue.

Step 11:

You will now be taken to the “Review and Submit Application” page.

  • Review all of the information you have entered to make sure everything is accurate.
  • If you need to correct anything, click the relevant field on the right side of the screen or use the “Back” button to navigate to the correct page and make your changes.
  • Once everything is verified, click “Submit” to send your application.

Step 12:

Once submitted, you will be brought back to the Home screen, where you will find a confirmation message indicating that your application has been received. The Massachusetts Medical Use of Marijuana Program will process your application based on the order it received and will inform you via email when a decision has been made.

Should your application be approved, you will immediately be able to print out a temporary Program ID Card. Your actual plastic Program ID Card will arrive by mail within 1-2 weeks. The temporary card expires 14 days after the date of approval.   • Navigate to the home page of your patient account. Once you are 60 days away from your registration expiration, you will be able to find the “Renew Registration” link at the bottom of the page. By clicking the “Renew Registration” button, you will start the registration renewal process.
  • Complete the registration renewal application by following the onscreen instructions. As all the registration information is already stored in your patient’s account, you will have to simply verify most of the data entered in your patient record initially. 
  • You will then proceed to the “Review and Submit Renewal” screen.
    • If anything requires correction in your details, then click on the corresponding box on the right side of the screen or press the “Back” button to go back to the particular page for corrections.
    • Upon completion of your checks, click on “Proceed” till you get back to the Review and Submit screen.
  • Once you confirm that everything is in order, click on “Submit.”
  • You will be brought back to the Home screen, and you will receive an acknowledgment that your renewal application has been submitted successfully. Your application will then be reviewed by the Program.
  • Once your application is approved, a new Program ID card will be mailed to you.
